        How to Use Superbills to Request Reimbursement from Insurance

        If I am not in-network with your insurance provider, you may still be able to get partial reimbursement for your sessions by submitting a 'superbill' to your insurance company. A superbill is a special receipt that includes all the information your insurer needs to process a claim for out-of-network benefits.

        1. Check Your Out-of-Network Benefits

        Before submitting a superbill, call your insurance provider using the number on the back of your insurance card. Ask the following questions:
        - Do I have out-of-network mental health benefits?
        - What is my deductible, and how much of it has been met?
        - What percentage of each session will be reimbursed?
        - How do I submit claims for reimbursement?
        - What is the mailing address or online portal for submitting claims?
        - Is a superbill sufficient, or do you require a specific claim form?

        2. Request a Superbill from Me

        At your request, I will provide you with a superbill-usually monthly. This document will include:
        - Your name and date of birth
        - My name, credentials, NPI number, and license number
        - Dates of service and session lengths
        - CPT codes for each session (e.g., 90834 for 45-minute therapy)
        - The fee charged and the amount you paid
        - Diagnostic code (required by insurance)

        3. Submit the Superbill to Your Insurance

        You can usually submit your superbill via:
        - Your insurance company’s online portal
        - Email (if accepted)
        - Mail (send to the address your insurance provides for claims)
        Be sure to include any required forms, if applicable.

        4. Wait for Reimbursement

        Insurance companies typically process claims in 2–4 weeks. You’ll receive reimbursement directly (via check or direct deposit), depending on your plan. If they deny the claim, they should send an Explanation of Benefits (EOB) with a reason. If needed, you can appeal their decision.

        Important Notes

        - Reimbursement depends on your out-of-network coverage.
        - You are responsible for paying the full session fee upfront.
        - I cannot guarantee reimbursement, but I will provide accurate documentation to support your claim.
